When IUI makes sense
IUI is simpler, cheaper, and less physically demanding. It's often the right starting point — but not for everyone.
Unexplained infertility with open tubes
If your HSG shows open fallopian tubes and there's no clear diagnosis, IUI with mild stimulation is a reasonable first step. It works by placing sperm closer to the egg, improving timing and odds.
Mild male factor
If your partner's sperm count or motility is slightly below normal (but not severely low), IUI can help by concentrating the best sperm and bypassing cervical barriers.
Under 35 with good ovarian reserve
Younger women with healthy AMH and AFC levels respond well to IUI. Success rates of 15–20% per cycle are realistic, and 3 cycles give you a cumulative 30–45% chance.
First-line treatment
Most evidence-based guidelines recommend trying 3–4 IUI cycles before moving to IVF — unless there's a specific reason to skip it. It's less invasive and costs 10–20x less.
Budget is a real constraint

When IVF makes sense
IVF is more expensive and physically demanding — but for certain diagnoses, it's the treatment with the best odds. Here's when going straight to IVF is clinically justified.
Blocked or damaged fallopian tubes
If both tubes are blocked, sperm can't reach the egg naturally. IUI won't help because it still relies on the tubes. IVF bypasses them entirely — eggs are retrieved directly from the ovaries.
Severe male factor infertility
Very low sperm count (under 5 million motile), poor morphology, or need for surgical sperm retrieval (TESA/MESA). IVF with ICSI — injecting a single sperm into each egg — is the standard approach.
Failed 3–4 IUI cycles
If you've done 3–4 well-timed, medicated IUI cycles without success, the data shows additional IUI attempts have diminishing returns. IVF offers a significant step up in per-cycle success rate.
Age 38 or older
Egg quality and quantity decline with age. At 38+, IUI success rates drop to 5–10% per cycle. IVF gives you better odds per attempt, and time matters — each year of delay reduces success rates further.
Endometriosis (moderate to severe)
Stage III–IV endometriosis affects egg quality, tubal function, and implantation. IVF success rates are significantly better than IUI for moderate-to-severe endometriosis.
Need for genetic testing (PGT-A)
If you have a history of recurrent miscarriages, known genetic conditions, or are over 37, PGT-A (preimplantation genetic testing) can screen embryos before transfer. This is only possible with IVF.

If the answer is yes — blocked tubes, severe male factor, your age, or a specific diagnosis — then IVF is the right call. If the answer is vague, or just “IVF has better success rates” without explaining why IUI won't work for your specific situation, it's worth asking more questions.
A good doctor will welcome this question. They'll explain the specific clinical reasoning for your case. If they can't, or won't, that tells you something too.
